Care Coordination Program
Care coordination is a patient-centered approach that involves working with the patient and family, primary care physician, specialists, and other members of the care team to create a support system that ensures the patient has the resources and tools to improve their health and quality of life. It involves organization of healthcare activities and collaboration between different healthcare providers to bridge the gaps between silos of care.
